What is CareEco?
CareOutreach uses advanced algorithms to unlock the patient data in your own EHR. CareOutreach connects to any EHR to scan patient records looking for gaps in care and then automatically engages patients to invite them to schedule their next step in their health journey. Practices can set up campaigns to target patient health needs and message via text or email to notify patients of their need for care. CareOutreach's online scheduling system connects directly to your practice EHR system to take much of the scheduling burden off of the staff. This results in more appointments for the practice which leads to patients getting the care they need, practices increasing their revenue, and quality scores improving thanks to the proactive preventative care system that CareOutreach provides practices.
Practices can enable as many or as few cohorts as they wish - including Annual Wellness Visits, Mammograms, Colonoscopies, Remote Patient Monitoring eligible patients, and much more.
