The platform provides hierarchical condition category (HCC) alerts on an annual/bi-annual basis that remind providers to document HCC-weighted Chronic Conditions for tracking purposes during a face-to-face encounters with patients. It also displays the RAF or Risk Score based on documentation found in the EHR.
With Veradigm EHR, clinicians can check their next-day schedules, review messages in their inboxes, or refill prescriptions while out of the office with Veradigm EHR Mobile. The platform also offers clinical images, an application built as a companion for Veradigm EHR, and integrated telehealth that engages patients in real-time video clinical consultations using a smartphone, tablet, or computer.
