Radekal is an electronic health record (EHR/EMR) solution designed to streamline the process of recording patient examinations and automatically generate history & physical examination (H&P) notes. The system can be used by Accountable Care Organizations (ACOs), medical practices, clinics, and hospitals, and solutions are available for a range of specialties, including Pediatrics, Obstetrics/Gynecology, Chiropractic, and Primary Care.

Radekal is designed to reduce the amount of time required by physicians to carry out and document patient examinations by streamlining the process of entering data. The system uses a range of taps/touches and swipes to mark pertinent positives and negatives, with users also able to add annotations during the exam or once the progress note is created. Swipes can be used to mark multiple fields in the review of system (ROS) or physical exam (PE) as positive or negative, so users only have to manually document any abnormal signs. Drill-down anatomical diagrams allow users to show the location of any symptoms or radiating pain. In-office procedures, diagnoses, and prescriptions can all be added at the end of the examination, and users can set up a ‘hot list’ in the Rx tab for quick access to commonly prescribed medications with prefilled information on the strength, dosage, frequency, interval, and treatment duration.

Radekal automatically generates H&P notes with full information on patient details, chief complaints, history of present illness, review of systems (ROS), Past, Medical, Family, and Social History (PMFSHx), active medications, allergies, physical exam and vital signs, assessment (diagnosis), lab orders, prescriptions, evaluation and management (E&M) codes, and counseling time. Coding is automatic at RAC auditable levels, and the unique progress notes are also designed to withstand RAC audits.
