Health Cloud Empower enables care providers, including care coordinators, hospice care, home nurses, and patients’ family members, to engage with care teams using any internet-enabled device. Care Team Lists allow caregivers to view and communicate with care teams, the Care Plan Selector enables the assignment of care plans to patients by caregivers, and Care Plan Tasks let care providers assign tasks to patients on-the-go. Permissions can be managed to control access to patient data, ensuring personal medical information is kept secure.

Concurrent Care Plans allow multiple teams with disparate expertise and workflows to collaborate and build relationships across care teams. Care providers can assign multiple care plans for comorbid patients to different care teams to provide more personalized care journeys. Health Cloud also automatically calculates patient risk scores based on the Center for Medicaid Services Hierarchical Condition Category (CMS-HCC) model, allowing providers to view risk across patient populations.
