What is Zentake?
Zentake is a paperless patient intake and consent platform for healthcare professionals. The software helps clinical support staff by streamlining front office tasks. It features an e-signature tool for collecting patient signatures via any internet-connected device. It securely stores Protected Health Information (PHI). Users can generate CSV or PDF files and combine multiple forms into single packets.
Front office staff can use Zentake to collect patient IDs and verify insurance before appointments. Medical data is collected through pre-filled forms, and staff can sign off on completed forms. Admins can set form restrictions or requirements and send emails or texts to patients in bulk or individually to request forms. Providers can also embed patient forms directly onto their website to streamline form-filling processes. The platform is a contactless document and consent option for patient-provider communications.

No intake form had the right stuff on it, and other intake software didn’t take into account that my state requires a therapist signature as well. Or they were super clunky/ugly/ not client end friendly.
